Inside the Value Revolution at Children’s Hospital Boston: Time-Driven Activity-Based Costing in Orthopaedic Surgery

چکیده
It is an exciting, and potentially turbulent time in the evolution of health care delivery in the United States. Despite decades of debate and piecemeal political reforms, the health care system remains inefficient in many ways and is becoming increasingly expensive. Increases in national health care expenditure continue to outpace inflation, exceeding 17% of the gross domestic product. Rising health care expenditures compromise both the economic stability of the United States and the treatment of the sick and injured. In addition, attempted regional and national health care reform has shouldered health care providers with further responsibility. The pressure is on providers to treat more patients, while offering better care at lower costs. “Better for less” is the demand. In this climate of change, physicians must educate themselves in the vocabulary of the epidemiological, political, ethical, and economic forces that influence the evolving health care economy, both to inform provider perspectives and to provide meaningful comment as reform progresses. Based on the seminal work of Harvard Business School Professors Michael Porter and Robert Kaplan in value-based health care delivery,2-5 the Department of Orthopaedic Surgery at Children’s Hospital Boston has initiated a series of projects aimed at improving value in health care delivery investigations specifically focused on reducing costs while maintaining or improving health outcomes.

Time Driven Activity Based Costing : Theory,Applications and Limitations
The aim of this study is to explore the strategic applications and limitations of Time-driven Activity-based Costing (TDABC) and to evaluate the degree of accuracy of the proponents’ arguments concerning its usefulness. In this study, published works directly related to this area from the period 2004-2015 are analyzed. This study reports TDABC's applications in strategic areas such as cost of p...
متن کاملOperating Room Time Savings with the Use of Splint Packs: A Randomized Controlled Trial
Background: The most expensive variable in the operating room (OR) is time. Lean Process Management is being used in the medical field to improve efficiency in the OR. Streamlining individual processes within the OR is crucial to a comprehensive time saving and cost-cutting health care strategy. At our institution, one hour of OR time costs approximately $500, exclusive of supply and personnel...
متن کاملAxillary Artery Injury Associated with Proximal Humerus Fracture: A Report of 6 Cases
Proximal humerus fractures are common, but associated injury of the axillary artery is uncommon. The majority of published blunt traumatic axillary artery injuries are associated with anterior glenohumeral dislocation; a few are associated with isolated proximal humerus fractures or fracture-dislocation. Experience within our institution demonstrates that axillary artery injury is often unreco...

Background: Geographic and doctor-to-doctor variations in care are a focus of quality and safety efforts in medicine. This study addresses factors associated with variation in the rate of operative treatment of de Quervain tendinopathy. Methods: We used a database including all patient encounters at 2 large medical centers, to study the experience of 10 hand surgeons and 1 physiatrist workin...
متن کاملDiscrete Pathophysiology is Uncommon in Patients with Nonspecific Arm Pain
Background: Nonspecific symptoms are common in all areas of medicine. Patients and caregivers can be frustrated when an illness cannot be reduced to a discrete pathophysiological process that corresponds with the symptoms. We therefore asked the following questions: 1) Which demographic factors and psychological comorbidities are associated with change from an initial diagnosis of nonspecific...
